Unsuccessful discharge reasons

Once a patient has completed its treatments at the clinic, the therapist can archive the current episode of care by discharging the patient.

3 options are available :

• Successful discharge
• Unsuccessful discharge
• Do not contact

If the clinic chooses the option Unsuccessful discharge, a dropdown menu with the available reasons will be displayed to add details before archiving the episode of care. 

Note : The reasons displayed in the dropdown menu have to be set up by the clinic.

Setting unsuccessful discharge reasons

To add reasons for unsuccessful discharge in the dropdown menu related to the button Unsuccessful discharge, go to :

1. Go to My Clinic
2. Click on the Tags tab
3. Click on Unsuccessful discharge reasons
4. Click on the Reasons for treatment plan not completed section on the left of the screen
5. Click on +
6. Add a reason
7. Click on Save Changes
