Forms Creator
With this functionality, you will be able to organize your forms such as health check-ups or active follow ups, but also modify them as you wish with an editor designed specifically for them.
Configuration and access to the module
An access right manages the display of the
Forms tab in the
My Clinic page, please configure this beforehand.
- Click on the My clinic in the top menu
- Navigate to the Users management tab
- Select the Access rights tab in the left submenu
- In the list of users, select the employee of your choice
- Click on the My clinic category
- Search for the Forms right and check it (or uncheck it to restrict access)
- Then navigate to the tabs on the My clinic page and click on Forms
List of forms
This page lists the forms of your clinic and allows you to access the modification of each one and organize it as needed with a folder system.
At the top right of the page:
- A dropdown menu Actions is displayed only if you select one or more forms from the list using the checkboxes. This gives you access to the form movement functionality. (learn more)
- A button to filter the list is displayed only the forms of type Active Follow Up or Health summary
- A button to create a new folder

A breadcrumb allows you to navigate the file tree to return to a previous folder.
Content of this list:
- Checkbox: (1st column) allows you to select the forms of your choice to move them to a folder.
- Type: indicates the type of the item (folder or form)
- Title: the title of the form
- Modification Date: the date of the last modification
- Employee: the name of the employee who modified the form
- Actions:
- On folders:
- clicking on the row or on the arrow to the right of the table allows accessing the content of the folder
- the pencil button allows modifying the name of the folder or to delete it (only if it is empty)
- On forms:
- clicking on the row opens the form in the form builder
- On folders:
Form Creator
The heart of the module, the form creator, allows you, as its name suggests, to customize your form according to your needs. A preview mode allows you to see the final rendering before saving, and the restoration of previous versions is also available.
Structure and functioning of the creator
Creative mode:
Creator's Menu:
- Form Status: represented by the blue color badge and can have two states: New Version and Edit, which are not important to the public.
- Form Title: a text field that allows you to freely edit the form title
- Actions: this dropdown menu allows:
- Add a Section: adds a new empty section at the bottom of your form
- Add a Signature: adds a new section at the bottom of your form with two fields: signature and date.
- Show/Hide Editor Toolbar: allows you to show or hide the toolbox to have more screen space or simply if you're not using drag and drop.
- Preview: allows you to switch to preview mode to preview the final result before saving your work.
Form Design Zone
The most important area of the module, because this is where the power of the editor comes to life. Each element to hover displays a menu with the allowed actions on the element.
- Insert: allows you to add an element before, inside, or after the element (defined by the type of element: block or field)
Insert before/after: allows you to add a block, i.e., a section or a section with a signature and date before or after the hovered element.

Insert in: allows you to add a field to the hovered element (text field, file, table, checkbox, etc).
- Copy: allows you to copy (without deleting the hovered element) and then paste it elsewhere.
- Cut: allows you to cut (delete the hovered element) and then paste it elsewhere.
- Paste: allows you to paste the previously copied or cut element at the desired location if the element allows it (refer to the difference between blocks and fields, further down in the Toolbox section).
- Duplicate: allows you to duplicate the hovered element the desired number of times (when duplicating a block, it will copy the entire block).
- Top / Bottom: allows you to move the hovered element to the top or bottom of the block it is in.
- Move Up / Move Down: allows you to move the hovered element one position up or down within the block it is in.
- Edit: allows you to edit the hovered element, the settings may vary depending on the element.
- Delete: allows you to delete the hovered element.
The drag and drop feature also allows you to rearrange sections or elements within sections for easy content management using the icon below each element.
Toolbox (Drag and Drop)
Two groups of elements are available in the toolbox:
- Blocks: these are the foundations of forms, such as sections and tables, that can contain field elements.
- Fields: these are different elements that can be added to blocks, such as text fields, file fields, dropdown menus, etc.
Both groups of elements can be dragged into the form design area using drag and drop. Here is an example below.
Note: drag and drop is not available for adding or moving elements within a table (temporary technical limitation).

Actions: this dropdown menu allows:
- Options: allows you to configure your form
- Health Assessment: checking this option will transform your form into a health assessment, remember to move it to a health assessment type folder to avoid it being hidden.
- Open Version History: opens a window that allows you to restore a previous version with a viewer to facilitate comparison between two versions and help you make your choice. (learn more)
- Move Form Location: opens a window that allows you to move the form to your folders. (learn more)
- Save: this button saves the changes and generates a new version of the form to keep a history of previous versions.
- Edit: allows you to switch to edit mode to modify the form.
ℹ️ Changes made to a form are not retroactive and will only apply to new activities created with this version.
Form preview area:
This area allows you to see the final rendering of the modifications you have made.
⚠️ No information entered in preview mode is stored in memory. So remember to do it in edit mode so as not to lose any information.
Move one or more forms
This window allows you to move one or more selected forms, either from the list of forms or from the creator.
Current location: indicates the current folder of the form(s)
New location: indicates the selected folder where the form(s) will be moved to (empty by default, as no folder is selected through checkboxes)
This list works like the list of forms, but only shows the folders, allows you to navigate through the tree structure, and allows you to select a folder using checkboxes. Once you have selected the desired folder, click
Save to move the form(s).
ℹ️ If you are using this feature from the creator, remember to save your form to apply the form location change.
Operation of versions for a form
Each time your form is saved, a new version of it is created in order to track and keep a history of the modifications made. This can also be used to restore a previous state through the window below, accessible in preview mode in the creator.
Previous version: This dropdown menu lists the existing previous versions. Select the version of your choice to display it next to the current version and view the differences.
After verifying both versions, you decide to restore a previous version. Click on
Save at the bottom of the window to confirm. A confirmation prompt will ask if you are certain about this action and explain what it implies.
ℹ️ Restoring a previous version does not affect existing activities and will only apply to new activities created with this restored version, which will become the current version.
Various questions
Can I break my form?
No, your form cannot be broken, you have the possibility to revert to a previous version if you wish.If you have any issues or don't know how to do something, contact Medexa support at
forms@medexa.com.
I would like to create a new form, when will this be available with this module?
This is planned, but in a future version. We need time to ensure that the current module is stable and functioning correctly. And meets your needs.
I would like to merge/split cells in a table, how do I do that?
Currently, this feature is not available to the public, as we are still working on fixes and testing for optimal functionality. If you need assistance with this action, contact Medexa support at
forms@medexa.com.
My form does not appear in the health summary ?
If you have selected Health Assessment in the form options, but it is not in a folder on the health assessment list, it will not be accessible. Contact Medexa support at
forms@medexa.com to restore access. (We are working on better management of this case for a future version).
Is drag and drop slow?
On large forms, drag and drop may be slow to respond due to the amount of information displayed on the screen and technical limitations we have. If you experience any issues, please wait a moment after a drag and drop. In case of major issues, save and close the form creator, then reopen the form.
Error when saving the form?
If you have an error message indicating that the backup has failed, please check that all fields do not exceed their character limit.
