How to submit a form ?
The Health Summary tab is used to record general documentation related to the patient. In particular, it allows you to send documents to be completed by your patients.
How to send a document to be completed by your patients?
To send a document to be completed, click on the (+) at the top right of the page and click on the Invitation button of the document concerned. Of course you can add a new document via the button Add a document (see below).
Here are the detailed instructions:
- Go to the Clients tab of the left menu
- Go to the Health Summary tab
- Click on the (+) at the top right of the page
- Click on Invitation next to the form to send
When clicking on the Invitation button, a new window will open
All you have to do is fill in the correct information and click on "Send invitation". Your patient will receive a new email (the time it takes to send the email may take longer or shorter, this depends on your email provider).
Note:
The forms located in this tab can be completed by the patient and/or the therapist. Here are some examples of forms that can be found in the Health Summary tab:
- Medical history
- Health Questionnaire
- Health check
- Consent form
Open and complete the document before sending it
As described above in the training on record keeping, you can send a document to be completed from the Health Summary tab of the patient file. However, it may happen that the patient has not completed the form for all kinds of reasons. So you can fill it with him in the software.
To do this, here are the indications:
- Go to the Clients tab of the left menu
- Look up a patient name
- Go to the Health Check tab
- Click on the (+) at the top right of the page
- Select the form to fill out
- Click on Open
All that remains is to fill in the necessary information and click on Complete or Save Changes.
How do I upload a document to the Health Summary tab ?
It is also possible to upload a document in the Health check tab.
For example, if your patient has completed their questionnaire by hand, it is possible for you to scan the questionnaire outside of Medexa and then upload it to the Health Summary tab of the patient file.
To download a document, here are the instructions:
- Go to Clients
- Look up a patient name
- Go to the Health Summary tab
- Click on the (+) at the top right of the page
- Click on Add document
- Click on Choose a file or drag the file
- Click on Save Changes
