Form Modifications/Corrections

All the modifications or corrections have to be sent to the following email address: forms@medexa.com


Modifications

The modifications are elements to add/delete/move/edit on your form.
The modifications are noticed by the clinic once they have used the form or by reviewing it right after its creation.
A modification is an element that hasn’t been clarified or was wrong detailed on the original version of the form that was submitted initially.

Note that we record the modifications in batches.
Therefore, is recommended to review all your forms and make sure all of your changes are counted before sending them to us.
If the modifications are sent on several submissions, charges may be applied for the future batches that are not included in your service agreement.

Corrections

The corrections are de mistakes that may have slipped into your forms when they were created.
The corrections are free at all times since the error comes from Medexa.
It is possible to make the distinction in between both of them looking at the original version of the form sent initially its the creation.


Here is 4 accepted option to submit your modifications/corrections:

Word

Clearly write your modifications to bring in a document Word.
We invite you to add screenshots to refer to areas affected by the modifications.
The annotations of any kind on these screenshots are appreciated if they help to clarify the changes requested.

Please, organize the information in different paragraphs specifying:
- Form’s name (as it is in Medexa)
- The section’s title affected
- The modification to bring

Email

Clearly write your modifications to bring in an email.
We invite you to add screenshots to refer to areas affected by the modifications.
The annotations of any kind on these screenshots are appreciated if they help to clarify the changes requested.

Please, organize the information in different paragraphs specifying:
- Form’s name (as it is in Medexa)
- The section’s title affected
- The modification to bring

PDF

Download the form in PDF format and indicate the modifications to bring with comments on the affected areas.
Please, make sure that you include in the comments the exact section where you would like to add/delete/edit.
Save the PDF with the comments you previously added and send it to us by email.

Scan

(This option is only possible for minor modifications. If there is an addition of major and/or repetitive text, this option is not considered.)

Print the form and write the modification by hand.
Please, make sure you write in printing letters, this way you assure that all annotations are clear and readable.
Please, make sure you well specify the exact place where you want to add/delete/edit any information to help yourself using arrows if necessary.
Once the modifications are well indicated on the paper version “scan” the form and sent it to do us by email.
